To resolve Fallout 3 performance issues on Windows 7 or 8, you should first ensure that you meet the game's minimum requirements and you have the latest NVIDIA or AMD graphics card drivers installed. Note: Fallout 3 is not optimized for Windows 7 and later operating systems.

Then make sure to delete the blank file you made. When Fallout 3 runs, it'll auto-generate appropriate ini's then. If using the Steam version, it's because Fallout 3 still insists on GFWL and won't even start without it, so it won't generate the Fallout.ini, which for some reason.
When I deleted the.ini files and ran the fallout 3 launcher, the same.exe files were placed back into the folder mentioned above. What I had to do was to delete the.ini files after I ran the launcher but before I started the game. Now I have a fallout.ini file with today's date and the game runs.
Check your Display Settings
Issues associated with launching and playing the game can often be resolved by changing the following display options:
Launch the game in Windowed Mode by selecting Options from the Fallout 3 Launcher and ticking the Windowed box under Mode.
If using dual monitors, disable your second one from your Windows display settings.
Ensure that the resolution of the game matches that of your display monitors by selecting Options from the Fallout 3 Launcher and selecting the correct resolution under Graphics Adapter and Resolution.
Right click the Fallout 3 application file and select Properties.
Select the Compatibility tab and tick the Run this program as an administrator box.

Fallout 3 was not developed for use on PCs with more than two cores. If your PC has more than two cores, please also try the following steps:
Open your My Documents folder.
Under My Games, open the Fallout 3 folder and select the fallout.ini file.
In the fallout.ini file, find the following line: bUseThreadedAI=0.
Change this line to read: bUseThreadedAI=1.
Add another line after it that reads: iNumHWThreads=2.
Run the game.
